Scheduling Your First Visit
1. Review the Membership Agreement
If you’d like to review the membership agreement before scheduling your first visit, you may preview it here:
Please note: The official agreement and required consents will be assigned through the patient portal after your first visit is scheduled and can be reviewed and signed electronically at that time.
2. Schedule Your New Patient Visit
When you’re ready, schedule your visit through the Charm Health scheduling system. Choose one of the following visit types:
New Patient In-Office Visit
New Patient Virtual Visit
You’ll see a space to briefly note what you would like the visit to focus on.
New patient visits are typically 60–90 minutes, allowing time to slow down and fully discuss your goals and concerns.
3. Set Up Your Patient Portal
After you submit your appointment request, I will personally review and confirm your visit.
You’ll then receive an email with instructions to create your Charm patient portal account.
I recommend setting this up right away so you can:
Complete intake forms
Upload prior labs or records (if desired)
Send secure messages
Access your visit summary after your appointment
After your visit, I typically share a written summary through the portal outlining what we discussed, next steps, and follow-up recommendations — so you have everything in one place to reference later.
4. Download the Charm App (Optional)
Many patients find the Charm Health app easier to use than a web browser.
You may download the app from the App Store or Google Play and log in using the same email you used to schedule your visit.
